Charlotte CHD has received its
shipment of seasonal flu.
Flu vaccines are available at all
three clinic locations by
appointment only.
Flu Shots: $ 25.00
Pneumonia Shots: $ 40.00
Acceptable forms of payment are: cash, check
and Medicare. We are unable to accept Medicare HMO’s or
Medicaid at this time.
YOU MUST BRING YOUR MEDICARE CARD
All Medicare HMO
enrolled are considered a "full pay" and will receive paperwork
to submit for reimbursement!!!
