Immunizations

The Charlotte County Health Department offers immunizations at its 3 clinic locations, and at various schools.

*If this your first time visit for immunizations, bring child's social security card and past shot records.*

Child must be brought in by custodial parent or have a written notarized (.pdf / 400.45kb) letter from the parent with a phone number for contact for that day along with complete immunization shot history.

PPD's (TB tests) are given Monday, Tuesday, Wednesday and Friday. Clients must return to the clinic two (2) days after test is administered for test results to be read.
